Lost or Damaged Indian Passport — Application

Complete the steps below. You can save a partially filled form at any time with Save & Finish Later and come back to it within 4 days.

⚠️Warning:

Your online document submission is monitored and recorded, including the IP address used at the time of submission. If fresh passport for minor, mention father's passport number
Contact no of a person living at your Indian address
For example: Customer service officer, Store Man, Retails Assistant, Branch Manager, Nurse, Chef, Payroll Officer, Process worker etc.

Indian Address

Provide their Adhar card or Passport first and Last page or lease agreement Or Electricity bill or Sales Contract or Gas bill or anything from Local govt/council/Panchayat that has full name and full address with PIN code on it.

Australian Address

Australian address - List of valid proofs (Upload any one)

  • Driver's License or Photo Id - Picture of physical copy only (Digital copy is not acceptable)
  • Electricity bill / Water bill / Gas bill
  • Rent agreement
  • Bank Statement is NOT acceptable
Any address proofs you provide should have the EXACT same address as the one you provide in the form.

Indian References

Provide details of two contactable references

  1. They should be living nearby your current Indian address (within approx. 20 km), not too far.
  2. Address of applicant, address of reference-1 and address of reference-2 (All 3 address) must be different.  (References living at same address are not acceptable.) However, if you are residing in a village area where house numbers are not allocated, the same address may be accepted.
  3. You must provide their full address including House No, Street, Area, City/Village, State and PIN Code.
  4. They can be your relatives such as Uncle, Aunty, Cousin or Friends/Neighbors who can vouch the identity.
  5. Immediate family members are NOT accepted, including:
  • Father / Mother
  • Brother / Sister
  • Spouse

Front and Back Both Pages. Proper scan copy preferred. If you are not able to scan, you can click clear pictures using your phone. Make sure all the details are clearly visible, photos are with high resolution, not blurry & the edges of document are not being cropped.
You must upload current visa copy.

PHOTOGRAPHS INSTRUCTIONS

Click 2-3 new photos of yours using your phone in different background & indoor/outdoor lighting

Most Important:

    1. Do not send selfie, old, edited, cropped, filtered, scanned, or printed photos.
    2. Any background is acceptable. Upload the natural photo only. Background and size will be adjusted by our office as per official requirements.
    3. Ask someone else to take the photo or use timer mode.
    4. Ensure the photo is clear and not blurry or low resolution.
    5. Stand straight, keep head straight and take a half-body photo (up to waist)
    6. Teeth should not be visible.
    7. Both shoulders and both ears must be clearly visible.
    8. No hair across ears. Please keep hair behind the ears or tie hair if needed.
    9. Avoid glasses, cap, earbuds
    10. There should not be too much shadow / light on any one side of the face.
    11. Whole face should have even lighting.
    12. We suggest you to turn your face towards the source of light.
    13. Try to click in natural lighting if possible.

(sign on plain white paper, click photo and upload, this sign will be printed on your passport, sign must match with current passport. If your signature is changed and you want your new signature to be printed on passport, please share photo of old & new both signatures.)

MOST IMPORTANT NOTE:

Please submit your application for now. Your spouse must submit a separate PASSPORT RENEWAL application through iVacs Australia.

If a separate application is not submitted for your spouse, only your PASSPORT RENEWAL application will be processed.

Details for PASSPORT RENEWAL of your Partner/Spouse

As your spouse's address to be printed on passport is the same Indian address as yours, the following details will remain the same

  1. Local Indian contact
  2. Indian address
  3. Police station
  4. References
  5. Australian address

Please provide all documents you have received related to your visas - from your first entry in Australia till date, which may include:

  • Visa Application Acknowledgement (when you applied)
  • Visa Grant Letters
  • Visa Refusal Letters / Decision Records
  • Any further visa applications after refusal
  • ART (Administrative Review Tribunal) review documents
  • Bridging Visa Grant Letters

For Damaged or Lost passport applications, two references are required. 

They must be currently living in Australia and holder of Indian passport.
